Please take a moment to review the following information to avoid delays in your registration.


Payment
Full payment must be received with your registration form.
Make checks payable to NASTD or charge your registration to American Express, VISA or MasterCard. Registrations without full payment will not be processed.

A purchase order may be used by State Members Only. A copy of the original purchase order must be faxed to 859-244-8001 before the registration can be processed.


REGISTRATION FEES
Full registration for the NASTD Western Region Seminar includes all program sessions and food functions indicated in the Program and evening social events.

Registrations will be accepted at NASTD Headquarters until June 2, 2006, 11:59 PM EDT.  After June 2 registrations will only be accepted on-site with payment made by check or credit card.  Registrations are not accepted by phone.

The cut-off for listing on the preliminary attendance list is June 2, 2006, 11:59 PM EDT.


REFUNDS
Written requests for registration refunds must be postmarked, faxed or e-mailed on or before June 2, 2006, 11:59 PM EDT
No refunds are made after June 2, 2006, 11:59 PM EDT.  Refunds are not given for no-shows.


CONFIRMATIONS
Registrations received by June 2 will receive confirmation via  e-mail with 5 to 7 business days.
