Payment
Full payment must be received with your registration form. NASTD accepts American Express, MasterCard or VISA. Registrations without full payment will not be processed.

 *PURCHASE ORDERS - STATE MEMBERS
A purchase order may be used by STATE MEMEBERS only. Please fax a copy of the original purchase order to 859-244-8001 to complete your registration.

REGISTRATION FEES
Full registration for the NASTD Midwestern Region Seminar includes all program sessions and food functions indicated in the program and evening social events.

Registrations will be accepted at NASTD Headquarters until May 3, 2010, 11:59 PM EDT.  After May 3, 2010 registrations will only be accepted on-site with payment made by check or credit card.  Registrations are not accepted by phone.

The cut-off for listing on the preliminary attendance roster is May 3, 2010 11:59 PM EDT.

REFUNDS
Written requests for registration refunds must be postmarked, faxed or e-mailed on or before May 3, 2010, 11:59 PM EDT.  No refunds are made after May 3, 2010, 11:59 PM EDT.  Refunds are not given for no-shows.
